Complete all items pertaining to the event (unless reserving only the date). Incomplete forms will be returned. Before submitting the Blue Form, it must be approved by the AOM director. Return it to the church secretary. This must be done no later than two (2) weeks prior to the event. If there are no conflicting events in the church schedule, the Blue Form will be presented at the soonest Thursday staff meeting for final approval. If you have any changes, contact the church secretary immediately. Changes must be approved again by the staff.
